Navigating disability services can feel overwhelming. This FAQ offers clear answers about our case management program, which supports individuals and families with guidance, advocacy, and personalized care for greater independence.

1. What kind of documentation do I need to bring to our meeting?

Please bring a copy of your ID. Additional documentation may be required based on program interests. Reach out to your case manager after an initial meeting for further details.

2. Does my loved one need a POA or ROI to meet with a Case Manager?

Yes, if you’re loved one is unable to communicate with a Case Manager a POA or ROI is required. Please feel to reach out to your case manager to request a ROI if needed.

There are three main ways to get on the Texas Medicaid Waiver list:

Currently there is not, however we anticipate to begin applying a fee at a later date on a sliding scale.

9. When can I expect to hear back from the case manager?

Within 48 hours of Request for services received.

10. How soon can I anticipate meeting with a case manager?

Scheduling can be done within a 1–2-week period depending on scheduling availability.
